We at OutbackGliders Sugar Gliders look to adopt our gliders out to loving forever homes. Since it is very important that a glider bonds properly in their new home, we require an adoption questionnaire to be filled out so we can learn more about you and answer any questions you may have. This helps us to ensure the glider you're interested in is an excellent match for you! We also require all families to fill out a contract upon adoption. While we wish that gliders never become unwelcome in their home, we understand that things can and do happen sometimes. To avoid any confusion or undo stress on the family or glider we require this contract. Simply put, if there comes a time when the glider needs to be rehomed we are the first to know, we get the first chance to claim that glider and/or will assist you in finding suitable home. You can not rehome the glider without my knowledge or approval.

 Please answer all questions with full honesty. If you have any questions, please do not hesitate to ask  us any questions!

1. Do you have Sugar Gliders Currently?, If so what are the ages and sex of the Glider/gliders?
2. What is your Cage sze? or intended cage size?
3. Do You know what color glider you’re looking for ?                                                                4. What is the diet you are feeding and why?
5. If you needed to re-home a glider that adopted from me what steps would you take?
6. Do you plan to breeding?
7. If you plan to breed, do you have another glider picked out for their mate?
8. Who is your veterinarian for your gliders (list name address and phone number please)
9. If your glider got sick who would you contact?
10. Are you an active member in the glider hobby i.e. are you a member of any forums? If so which ones and what screen name do you use?
11. Who all resides in your home (human and pets) list names ages breeds and how long you have had them (This is more about getting to know you than anything else).
12. Do you rent or own? Is your landlord (if you have one) aware of your gliders and are they allowed? 
13. Do you have any questions? Who do you usually go to if you have questions?  
14. What kind of toys and other things for stimulation do you have for the gliders?
